Main function of the job:

To maintain Care skills at a current level, and undertake such training and development as may from time-to-time be required to maintain practices as up-to-date.
To provide leadership to the Care functions of the Agency.
To organise and co-ordinate Care services in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager, and within the financial plans agreed from time-to-time.
To organise, coordinate and supervise the Care services within the Agency in accordance with agreed standards.

Supervisory responsibilities:
To organise, co-ordinate and supervise the deployment of the Care services within The Agency within the legal and contractual standards relating to the Service.
Main Duties (not in any order of priority)
Management of the organisation:
1. Develop effective working relationships with the Registered Manager of The Agency and the Office Manager
2. Support an open, positive and inclusive management culture.
3. Participate in the evaluation of The Agency against agreed organisational goals, as well as business and quality objectives.
4. Work to establish effective employer-employee relationships.
5. Minimise legal risks.
6. Participate in the maintenance of the Agency’s management information systems.
7. Assist in the formulation and implementation of Care policies and procedures.
8. Assist in the implementation and maintenance of the standards required by legislation related to the registration of the Agency, and other legal requirements.
9. Act within the Agency’s budget based on the Agency’s objectives and within the projected revenue.
10. Work in a cost-effective manner.
11. Be involved in the implementation and maintenance of the Agency’s quality assurance programme.
12. Assist in the design and administration of an evaluation of the Care standards and Care service provision.
13. Systematically solve day-to-day problematical issues which arise.
Job Description – Care Coordinator
Main Duties (not in any order of priority):
Management of Care Services:
1. Ensure that the Care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards.
2. Ensure that the staff allocated, and the staff attending, each Service User have the
relevant skills, knowledge and resources related to the opportunities and needs of that Service User.
3. Ensure that each staff member is introduced to the Service User before or at the
beginning of the first visit by that staff member, and that the Service User always knows who to expect at each visit.
4. Manage travel delays and staff shortages to mitigate to the fullest extent possible the
impact on the Service User, managing and minimising their distress and insecurity in such situations.
5. Assist in the development of the philosophy, goals and objectives for the Care practice.
6. Assist in the assessment of the effectiveness of Care implementation and delivery.
7. Implement action to meet and maintain Care standards.
8. Work in cooperation with members of multi-disciplinary health teams so as to maximise opportunities for Service User therapeutic Care.
9. Ensure that the Care service is delivered in a safe and person-centred manner.
10. Ensure that Service User rights are protected.
11. Encourage a model of self-care and Service User rehabilitation.
12. Evaluate Care Plans.
13. Evaluate standards of Care competence.
14.Promote the organisations brand
15.Develop and manage marketing campaigns
Professional Long Term Care Leadership:
1. Encourage innovative methods for the delivery of Care.
2. Encourage health promotion within Care strategies.
3. Seek opportunities for personal and professional growth.
